The role

This is an exceptional opportunity for a motivated NP/PA to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network. The Advanced Heart Failure APP will work as part of a multidisciplinary team providing comprehensive care to patients with heart failure, including those receiving guideline-directed medical therapy (GDMT) and advanced heart failure therapies. This role is primarily involves close collaboration with cardiologists, nurses, pharmacists, and care management teams to optimize patient outcomes and reduce hospitalizations.
